Statement of Purpose


Our Mission

We are proud of our ethos of patient friendly care, where the patients get to know their doctor and the doctor gets to know the patient. This traditional approach has enabled our general practice to continue to provide holistic care and practise family medicine in the best possible sense.

Our doctors work very closely with patients and even visit certain elderly patients in hospital when they are admitted, again, to ensure continuity and liaison between hospital and primary care.

Aims and Objectives

  • To provide traditional, yet high-quality family medicine in a friendly and relaxed environment.
  • To continue to provide a one-to-one approach where patients can be seen by the same doctor of their choice.
  • To ensure continuity of care, patients can see the doctor of their choice and develop a relationship with that doctor.
  • To offer a wide range of services that meet the needs of the practice's registered patients.
  • To offer independent services that meet the needs of patients at the practice and other people who live in the surrounding area.

 Doctor Hafiz Oak Glen Surgery palliative and elderly care specialist

Dr. Aziz Ahmad Hafiz, MBBS, MRCGP 

GP Partner

Dr. Aziz Hafiz qualified with MBBS 1999 London MRCGP and joined the practice in January 2007 as GP partner. He undertakes 6 sessions per week and is responsible for the training of GP trainees and students from Leeds University. Dr Hafiz undertakes basic minor surgery at the practice. He is practice lead for practice based commissioning, finance and GP education and training. Dr Hafiz's special interests are in palliative care, elderly care and GP education. Dr Hafiz also speaks Urdu.

Dr. Sarah Lucy Whitfield, MBBS, MRCGP, DRCOG, DFFP

GP Partner

Dr. Sarah Whitfield qualified with a MBCHB 1997, Leeds MRCGP & DRCOG and joined the practice in October 2009 as GP partner. Dr Whitfield undertakes 4 sessions per week. Dr. Whitfield is responsible for the training of GP trainees. She is the lead for sexual health, prescribing and safeguarding children for the practice. Dr Whitfield's special interests are sexual health, women's problems and elderly medicine.

Dr. Alison Brown, MBBS

Salaried GP

Dr. Alison Brown qualified with MBBS (honours with distinction) in 1998 and joined the practice in October 2014 as salaried GP. Dr Brown undertakes 3 sessions per week on Thursday and Friday afternoons. Dr. Brown is also a clinical lecturer at the University of Leeds.

Sister Stephanie Taylor

Practice Nurse

Sister Stephanie Taylor completed her BSc (Hons) in Nursing Studies (Primary Health Care) in July 2010. 'Steph' is our Practice Nurse and runs the many chronic disease clinics including diabetes, asthma, COPD, hypertension, coronary heart disease etc. Her special interests are in diabetes, respiratory health, women's health, health promotion and travel immunisation.

Vanessa Bradley is our HCA and she runs the phlebotomy clinic as well as assisting Sister Taylor with various nursing responsibilities.

Angeline Nicholson

Practice Manager

Mrs. Angeline Nicholson qualified with a BA (Hons) in Business Studies in 2000 and became a member of the Chartered Institute of Personnel and Development in 2003. She joined the practice in 2007. She is responsible for managing finances, human resources, practice-based commissioning, building management, statistical reporting any administrative or non-medical problems, and the smooth running of the practice. She is available to discuss provision of services, suggestions or complaints.

Caroline is our secretary and deals with a wide range of administrative duties.
